THE ELDERLY PERSON AFTER BEING DISCHARGED: VERIFYNG THE CONTINUATION OF HOME CARE

Whatever the proposal might be improve the quality of assistance, it is vital to implement different evaluation methods. In view of that, the presen! study proposes to evalúate, after implementing the discharge planning for a group of 50 elderly women hospitalized in the emergency ward, the continuation of home care from the 6 th to 8lh day after being discharged. The analysis of the results verified that the mayority of the elderly woman and caregiver were correctly following many of precautions proposed in the descharge coordination. Sometimes, however, it was found necessary for a professional to accompany the changes in the elderly women's health status, once they are presented themselves very vulnerable to complications.
